The forms can be accessed in three different formats.
  1. Omniform Internet Filler extension (fml)
  2. Adobe Acrobat Reader, 5.0, or Adobe Acrobat Approval 5.0 extension (pdf)
  3. Microsoft Word extension (doc)
The Omniform program functions with Mac, Windows 95 and 98 and Netscape.
The Adobe Acrobat functions will all formats.
If you can access the Omniform Filler it is recommended that you use it.
There are some differences in the programs.
  1. The Omniform Filler, can be filled out on line, and the forms, with data can be saved on your computer for later revision. However Omniform has stopped updating this program and there are fewer and fewer platforms available for it.
  2. The Adobe Acrobat (reader) can be filled out on line and all calculations are automatic. You can save the form on you computer, but you cannot save the data unless you purchase Adobe Acrobat approval 5.0 for $39.00. The program is an excellent program that will be useful to you in other settings. Legal Line does not profit from it's sale.
  3. Lastly there is Microsoft Word. It can be downloaded to your hard drive and filled out off line. There are no automatic calculations as of this date, but upgrades will be commencing soon.

         Tthe Omniform Internet FillerTM is used to access the forms which are provided. Before it can be used you must down load the plug in.

  • The oif32 is used if you are using WindowsTM95, 98 or NT,
  • the oif16 is used for WindowsTM 3.1 and
  • theMacTM is mac_internet_filler.hqx
  • IF YOU ARE USING NETSCAPE 6 THEN YOU MUST GOT TO Netscape 6 support page for further instructions.

  •         After the file is downloaded run the exe or hqx program and follow the instructions to install the program. MAKE SURE YOUR BROWSER IS CLOSED WHEN YOU INSTALL THE PLUG IN.



    Make sure to check "setup for use as a Separate Helper Application" that will allow you to use the program without the use of your Browser.
            Once you have the plug in installed you can start your browser. In some instances you may have to re-boot your machine, do a hard boot by shutting down your computer and restarting it.

            Okay you are ready, you can go to the forms page and click a form. After the form is opened you will notice the menu bar for the Omniform Internet fillerTM

    Be sure to notice the "Previous" and "Next" on the menu bar. This is the way you move back and forth to other pages of the form. You can save your completed forms to your hard drive, by clicking the

    "File" and then "Save" button you will be asked where to save it, and you can select, or create a file on your hard drive, or floppy.

            You can open the form by going to your form and clicking it. The Omniform FillerTM will open the form. If you have trouble opening the form in this manner you can open your browser, open the form by going to "file" and clicking "open page".

            You will find that some forms have two pages. On those forms the court requires that they be printed on front and back with the top to the bottom. To print you need to print the first page, take the paper turn it upside down and backward and place it back into the printer to print the second page.

            On some occasions you may find, when opening a form, that you are kicked off of the Internet. This does NOT have anything to do with the form, but instead with your INTERNET SERVICE PROVIDER. One way to avoid this problem is to go to the form link, click your right button on your mouse, and save the fml form to your hard drive. This allows you to open the form and use it off line.
